بیانِ حلفی برائے گمشدگی

منکہ ____________ ولد ____________، شناختی کارڈ نمبر ____________، ساکن ____________، حلفاً بیان کرتا ہوں کہ:

1۔ یہ کہ میرا ____________ (شناختی کارڈ / سند / رجسٹریشن بک) نمبر ____________ مورخہ ____________ کو بمقام ____________ کہیں گم ہو گیا ہے۔

2۔ یہ کہ کافی تلاش کے باوجود مذکورہ دستاویز نہیں مل سکی۔

3۔ یہ کہ مذکورہ دستاویز کسی ادارے یا شخص کے پاس بطور ضمانت یا رہن جمع نہیں ہے۔

4۔ یہ کہ اگر اصل دستاویز مل گئی تو متعلقہ دفتر میں جمع کرا دوں گا، اور اس کے غلط استعمال کی صورت میں تمام تر ذمہ داری میری ہو گی۔

میں حلفاً بیان کرتا ہوں کہ مذکورہ بالا بیان میرے علم و یقین کے مطابق درست ہے اور اس میں کوئی امر پوشیدہ نہیں رکھا گیا۔
