When is an affidavit for a lost passport needed?

A passport that is lost or stolen has to be reported and replaced, and the passport office will not issue a new one on the application alone. It asks for the police report and a sworn statement of how the passport was lost, declaring that it has not been pledged, sold or kept by anyone.

Where it goes

Inside Pakistan it goes with the application at the regional passport office. Abroad, the loss is reported to the local police and the application is made at the Pakistani embassy or consulate. Replacing a lost passport takes more checking than a renewal, and the fee is usually higher.

Points to get right

  • Report the loss to the police first and keep the report. Its number goes into the application.
  • Write the passport number and its dates of issue and expiry if you have a copy. Keep photocopies of passports for this reason.
  • Say where and when it was lost as exactly as you can.
  • If the old passport turns up after a new one is issued, hand it in. Using it is an offence.

What to fill in

  • The name, father’s name, CNIC number and address of the person making the statement
  • The facts, each in its own numbered line
  • The date, and the signature or thumb impression
  • The attestation of an oath commissioner, where the office asks for it

These are general formats to save typing. Requirements differ between offices, courts and provinces, and a paper about property or money has legal effect. Have the final wording checked by a deed writer or lawyer, use stamp paper of the correct value, and get the paper attested or registered where that is required.

What do I need to replace a lost passport in Pakistan?

Normally the police report of the loss, an affidavit, your CNIC, and a copy of the lost passport if you have one. The passport office lists its current requirements.

What if my passport is lost abroad?

Report it to the local police, then apply at the nearest Pakistani embassy or consulate, which can issue a new passport or an emergency travel document.
