بیانِ حلفی برائے درستگیِ نام

منکہ ____________ ولد ____________، شناختی کارڈ نمبر ____________، ساکن ____________، حلفاً بیان کرتا ہوں کہ:

1۔ یہ کہ میرا درست نام ____________ ہے جو میرے شناختی کارڈ میں درج ہے۔

2۔ یہ کہ میری ____________ (سند / دستاویز) میں میرا نام غلطی سے ____________ درج ہو گیا ہے۔

3۔ یہ کہ مذکورہ دونوں نام ایک ہی شخص یعنی من مظہر کے ہیں۔

4۔ یہ کہ میں اپنے ریکارڈ میں نام کی درستگی کروانا چاہتا ہوں اور اس پر کسی کو کوئی اعتراض نہیں۔

میں حلفاً بیان کرتا ہوں کہ مذکورہ بالا بیان میرے علم و یقین کے مطابق درست ہے اور اس میں کوئی امر پوشیدہ نہیں رکھا گیا۔

The attestation of an oath commissioner, where the office asks for it
